Job Description

Serenity, Inc. is seeking a compassionate and dedicated Ryan White Non-Medical Case Manager to join our team in Petersburg, VA. This position focuses on providing comprehensive support to clients living with HIV, addressing their psychosocial and health-related challenges. The Non-Medical Case Manager will work closely with clients to ensure they have access to necessary services and support systems.

Key Responsibilities 

  • Client Services: Provide direct services to Ryan White clients in community, clinic, and home settings according to need and established program protocols.

  • Care Coordination: Coordinate medical care and services, including referrals to healthcare, psychosocial, substance use, and other community services.

  • Client Monitoring: Conduct face-to-face assessments and reassessments, develop individualized service plans, and monitor client progress.

  • Health Education: Educate and counsel clients about HIV transmission, disease management, harm reduction, partner services, and medication adherence.

  • Support Services: Assist clients with insurance applications, housing, food, and transportation referrals.

  • Documentation: Maintain accurate and complete client files, treatment plans, and program databases.

  • Community Engagement: Develop relationships within the community to access people living with HIV and high-risk individuals, and conduct presentations and trainings.

Qualifications & Skills

Qualifications:

  • Education: Bachelor’s degree in social work, public health, or a related field preferred.

  • Experience: Previous experience in case management, healthcare, or social services, particularly with HIV/AIDS populations.
