Serenity, Inc. is seeking a dedicated and compassionate Community Health Worker (CHW) to join our team in Petersburg, VA. This full-time, grant-funded position focuses on health promotion and the prevention of HIV/STIs within the community. The CHW will play a crucial role in improving the health outcomes of community members through education, advocacy, and support.
Key Responsibilities
Outreach and Engagement: Conduct outreach activities to engage community members, including home visits, workshops, and community events.
Health Education: Provide culturally sensitive health education on HIV/STI prevention and general health promotion.
Care Coordination: Assist clients in navigating the healthcare system, including scheduling appointments and connecting them with necessary services.
Advocacy: Advocate for clients to ensure they have access to public health programs and resources.
Support Groups: Facilitate support groups and educational sessions on relevant health topics.
Record Keeping: Maintain accurate and confidential records of client interactions and services provided.
Social Media Campaigns: Create and manage social media campaigns to promote health communications and engage the community.
Health Communications: Utilize digital tools and platforms to effectively disseminate health information.
Qualifications & Skills
Qualifications:
Education: Associate’s degree in a health-related field preferred. Certification as a Community Health Worker is a plus.
Experience: Previous experience in healthcare, social services, or community outreach is preferred.

Communication: Strong verbal and written communication skills to effectively interact with clients and healthcare professionals.
