Job Description

Serenity, Inc. is seeking a dedicated and compassionate Community Health Worker (CHW) to join our team in Petersburg, VA. This full-time, grant-funded position focuses on health promotion and the prevention of HIV/STIs within the community. The CHW will play a crucial role in improving the health outcomes of community members through education, advocacy, and support.

Key Responsibilities 

  • Outreach and Engagement: Conduct outreach activities to engage community members, including home visits, workshops, and community events.

  • Health Education: Provide culturally sensitive health education on HIV/STI prevention and general health promotion.

  • Care Coordination: Assist clients in navigating the healthcare system, including scheduling appointments and connecting them with necessary services.

  • Advocacy: Advocate for clients to ensure they have access to public health programs and resources.

  • Support Groups: Facilitate support groups and educational sessions on relevant health topics.

  • Record Keeping: Maintain accurate and confidential records of client interactions and services provided.

  • Social Media Campaigns: Create and manage social media campaigns to promote health communications and engage the community.

  • Health Communications: Utilize digital tools and platforms to effectively disseminate health information.

Qualifications & Skills

Qualifications:

  • Education: Associate’s degree in a health-related field preferred. Certification as a Community Health Worker is a plus.

  • Experience: Previous experience in healthcare, social services, or community outreach is preferred.

  • Communication: Strong verbal and written communication skills to effectively interact with clients and healthcare professionals.

  • Interpersonal Skills: Ability to build trust and rapport with diverse community members.

  • Empathy: Demonstrate compassion and understanding towards clients' needs and challenges.

  • Community Knowledge: Familiarity with the Petersburg community and its resources.

  • Technology Proficiency: Comfortable using technology, including social media platforms, digital communication tools, and health information systems.

  • Organizational Skills: Strong organizational skills to manage multiple tasks and maintain accurate records.

  • Problem-Solving: Ability to identify issues and develop effective solutions.

  • Cultural Competence: Sensitivity to and respect for cultural differences in the community.
