Job Description

Serenity, Inc. is seeking a compassionate and organized Intake Coordinator to join our team in Petersburg, VA. The Intake Coordinator will be responsible for managing the initial intake process for new clients, ensuring they receive the necessary support and resources. This role is crucial in helping clients access the services they need to improve their health and well-being

Key Responsibilities 

  • Client Intake: Conduct initial assessments and gather detailed information from new clients to determine their needs and eligibility for services.

  • Documentation: Maintain accurate and up-to-date records of client information, ensuring confidentiality and compliance with privacy regulations.

  • Coordination: Work closely with case managers and other staff to coordinate services and referrals for clients.

  • Communication: Serve as the primary point of contact for new clients, providing them with information about available services and resources.

  • Follow-Up: Conduct follow-up calls and meetings to ensure clients are connected with the appropriate services and support.

  • Support Services: Assist clients with completing necessary paperwork and applications for services such as insurance, housing, and food assistance.

  • Community Engagement: Develop and maintain relationships with community organizations and service providers to facilitate client referrals and resource sharing.

Qualifications & Skills

Qualifications:

  • Education: High school diploma or GED required; associate’s or bachelor’s degree in social work, public health, or a related field preferred.

  • Experience: Previous experience in a healthcare, social services, or customer service role is preferred.
